When you choose Fairhaven for community or residential supports, we match the workers skillset, to suit the participant.

Our diverse team of support workers are all highly skilled professionals who value and respect people’s abilities. We can partner you with someone who likes the things that you like, has a similar personality and interests and/or has skills in something that you might like to try doing.

What are your goals?

School Leaver Employment Support (SLES)

SLES is an early intervention approach for people living with disability who are in their final year of school. SLES funding may be available as part of an NDIS Plan for up to two years only. It is available to support the transition to post-school pathways

SLES can assist you through unpaid work experience, to overcome barriers, build confidence, and develop an understanding of a working lifestyle – valuable life skills that will help you become work ready.

The amount of support you receive will depend on your needs and goals. SLES supports in your NDIS Plan could include:

  • Work experience in open employment
  • Specific job skills training
  • Communication skills
  • Understanding employer expectations

As a SLES provider, we can help with your employment goals by seeking the right work experience, skills and training for you.

If you are looking at post-school options and do not have SLES funding in your NDIS Plan, one of our highly skilled team can help get you started.

Support Coordination

When you first receive your NDIS Plan and budget it can be overwhelming to know where to begin.

Support Coordination can help you to put your NDIS Plan in to action; coordinating and implementing your supports, linking you with the best value providers, show you how to use the myplace online portal and more.

A good support coordinator will work closely with you to build your skills and confidence in providing this service until eventually you require little to no support coordination at all.

Funding for ‘Support Coordination’ must be requested at the time of planning, and may be included in your NDIS Plan where it is deemed reasonable and necessary.

Work experience

If you are curious to learn more about working at one of our supported employment enterprises, come and do some work experience to see first-hand what it is really like.

You will get to choose which area you would like to work in and the type of work you would like to do. We encourage you to try out as many different areas and tasks as you like. Then you can be sure that this is the right environment for you.

Work experience is available to people with and without NDIS funding.

Prepare for NDIS planning

It’s never too early to get ready for your NDIS planning conversation.

Start making a list of your short-term goals and gather up any health care records. Also get a copy of your first Service Agreement that will show your outcomes from the last 12 months if you are already an NDIS participant.

If you have any goals that are different to the previous year, think carefully about what supports might be necessary to achieve your new goals. It’s a good idea to bring along any documents to validate this, such as service quotes.

Travel training

If your goal is to be more independent and confident when getting around from A to B, funding for Travel Training can help you achieve this. This can include travelling between home, work, shopping, the city and other places, as well as visits with friends or family.

Funding for Travel Training support can provide:

  • Advice on how to plan your trips so you are more relaxed
  • Learning to understand bus, train or ferry timetables
  • A companion when travelling to help you gain confidence

Travel Training can provide you the preparation and practice you need to increase your independence when accessing the community.

Support for social activities

Access places and activities in the community either through one-on-one support or as part of a group of people. Get involved with a social network, meet people, make friends, develop better social skills and build stronger relationships.

Choose to attend an event, join a class, be part of a team, play a game or just hang out. We can offer a variety of social, recreational or leisure activities to suit your goals and interests.

Meal planning and cooking

Learn how to prepare a meal with little or no assistance, or try out a new recipe. Cook for yourself, friends or family.

We provide support around meal planning and cooking with consideration to your budget, tastes and dietary requirements. Meal planning support can include:

  • Deciding the type of meals you would like to cook
  • Working out what ingredients you may already have, or will need to buy
  • Making a shopping list
  • Working out how much food costs, budgeting, learning ways to find the best value when shopping
  • A shopping trip to buy food
  • Safety and hygiene when preparing food and using equipment
  • Cooking techniques
  • Doing the dishes and clearing away after a meal

We can help you develop routines around planning, shopping and preparing food.

Get involved with great activities

If you are keen to get involved but not sure where to start, try one of our activities. We regularly meet up around different places on the Central Coast, including parks, bush walks, the bowling alley, movies, laser tag, shopping trips and lunch at the local club, as well as bus trips to Hunter Valley or travel to Sydney.

Our Point Clare site has a Community Hub which offers, at varying times, a range of indoor activities such as craft, games or drumming.

Improve basic literacy and numeracy

Feel your independence and confidence grow when you improve your basic reading, writing and number skills!  Learn about basic maths, words, shapes and counting. Get assistance to:

  • Read maps
  • Manage your budget
  • Write letters
  • Keep a journal
  • Read the paper

Telling time

With so many digital devices around it is easy to think that people can get by if they have not learned to tell the time on a clock with hands. But clocks are still very much in use in lots of places. Being able to read the time on a clock face is a great skill to have.

It is never too late even as an adult to learn how to tell the time. We can help you achieve this with individual support or as part of a group.

Live independently in your own home

Whether you need someone to just drop in once a week for a few hours or something more frequent, we can tailor a plan to support you with daily tasks in your own home. Types of tasks might be:

  • Meal planning and preparation
  • Shopping
  • Support with travel training
  • Personal care and grooming
  • Housework
  • Planning a routine
  • Managing health care appointments and medication
  • Gardening
  • Entertaining friends and family

Funding for these supports these might come under the following categories in your NDIS Plan:

  • Assistance with daily life at home, in the community, in education and at work
  • Improved daily living skills
  • Improved living arrangements

 

Live independently in a Fairhaven residence

Finding your own place to live can be fun and rewarding, and take some getting used to. If your goal is to live independently, we can help you get settled in one of our shared living homes.

With a shared living lifestyle, you can design the perfect blend of independence and personalised support. We have two supported accommodation sites; Kincumber and Booker Bay.

You can choose drop in support or something more hands-on. Our support staff are available 24 hours a day to accommodate your lifestyle needs.
